Prostacel is administered as an injection directly into the corpus cavernosum (the erectile tissue of the penis). It is highly effective for men who do not respond to oral ED medications such as sildenafil. The erection typically occurs within 5 to 20 minutes and lasts for about 30 to 60 minutes, depending on the individual response.

In neonates with congenital heart defects, Alprostadil is used as a temporary treatment to maintain the patency of the ductus arteriosus, improving blood circulation until surgical correction can be performed.

Common side effects of Prostacel include mild pain at the injection site, prolonged erection (priapism), or a slight burning sensation in the penis. More severe side effects, though rare, may include penile fibrosis (scar tissue) or systemic effects like dizziness or low blood pressure.
